Liverpool children's hospital Alder Hey has fully admitted responsibility for 24 failings in care that led to the death of 21-month-old Eleanor Hazel Aldred-Owen from Mold (Wales) in October 2023. The coroner concluded that the death resulted from misadventure aggravated by neglect: an abnormal chest X-ray was not passed on to doctors, and the collapsed lung was identified too late.
